Initial Comments:


Based on the findings of an onsite unannounced relicensure survey completed July 21, 2021 Good Brothers Home Health Care, was found to be in compliance with the requirements of PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart A, Chapter 51.

Based on the findings of an onsite unannounced state relicensure survey, completed July 21, 2021, Good Brothers Home Health Care was found NOT to be in compliance with the following requirements of PA Code, Title 28, Health and Safety, Part IV, Health Facilities, Subpart H, Chapter 611, Home Care Agencies and Home Care Registries.

Direct Care Worker Files

Files for direct care workers employed or rostered shall include documentation of the date of the face-to-face interview with the individual and of references obtained. Direct Care Worker files also shall include other information as required by § 611.52, § 611.53, if applicable, § 611.54, § 611.55 and § 611.56 (relating to criminal background checks, child abuse clearance, provisional hiring, competency requirements; and health evaluations).

Observations:


Based on review of facility policy, employee files, and interview with the Administrator, it was determined that the agency failed to document a face-to-face interview with the individual, and to obtain two verifiable references that affirms the ability of a Direct Care Worker to provide home care services for eight (8), of 10 employee files reviewed. (DCW #2, 3, 4, 5, 6, 7, 8 and 10).


Findings include:


Review of personnel files on July 21, 2021 at approximately 12:00 p.m. revealed that DCW #2, was hired on March 10, 2021. There was no documented evidence that a face-to-face interview was conducted. A further review revealed no documented evidence that one or two verifiable references were obtained by the facility.

Review of personnel files on July 21, 2021 at approximately 12:15 p.m. revealed that DCW #3, was hired on May 26, 2021. There was no documented evidence that a face-to-face interview was conducted. A further review revealed no documented evidence that one or two verifiable references were obtained by the facility.

Review of personnel files on July 21, 2021 at approximately 12:30 p.m. revealed that DCW #4, was hired on July 7, 2021. There was no documented evidence that a face-to-face interview was conducted. A further review revealed no documented evidence that one or two verifiable references were obtained by the facility.

Review,w of personnel files on July 21, 2021 at approximately 12:45 a.m. revealed that DCW #5 was hired on April 17, 2021. There was no documented evidence that a face-to-face interview was conducted. A further review revealed no documented evidence that one or two verifiable references were obtained by the facility.

Review of personnel files on July 21, 2021 at approximately 01:00 p.m. revealed that DCW #6, was hired on June 9, 2021. There was no documented evidence that a face-to-face interview was conducted. A further review revealed no documented evidence that one or two verifiable references were obtained by the facility.

Review of personnel files on July 21, 2021 at approximately 01:15 p.m. revealed that DCW #7, was hired on May 21, 2021. There was no documented evidence that a face-to-face interview was conducted. A further review revealed no documented evidence that one or two verifiable references were obtained by the facility.

Review of personnel files on July 21, 2021 at approximately 01:30 p.m. revealed that DCW #8, was hired on May 21, 2021, There was no documented evidence that a face-to-face interview was conducted. A further review revealed no documented evidence that one or two verifiable references were obtained by the facility.

Review of personnel files on July 21, 2021 at approximately 02:00 p.m. revealed that DCW #10, was hired on May 17, 2021. There was no documented evidence that a face-to-face interview was conducted. A further review revealed no documented evidence that one or two verifiable references were obtained by the facility.

During an exit interview with the agency administrator on July 21, 2021 at approximately 2:15 p.m., it was confirmed that agency had failed to document face-to-face interviews, and to obtain two verifiable references for each DCW employed.







Plan of Correction:

Although face-to-face interviews were conducted with all employees of Good Brothers Home Health Care, it was not documented clearly to those not familiar with our agency. We have created a separate and specific form that details when each face-to-face interview was conducted with each employee hired.
There will also be separate forms for each employee documenting their two references and the information that was shared with employer by each employee reference. Both of these corrections will be applied to all current and future employees.

(1) The photo identification tag shall include a recent
photograph of the employee, the employee's first name, the
employee's title and the name of [the health care facility or
employment agency.] any of the following:
(i) The health care facility.
(ii) The health system.
(iii) The employment agency.
(iv) The fictitious name of an entity under
subparagraph (i), (ii) or (iii) which is registered with
the Department of State under 54 Pa.C.S. Ch. 3 (relating
to fictitious names) or a successor statute.

(2) The title of the employee shall be as large as possible
in block type and shall occupy a one-half inch tall strip as
close as practicable to the bottom edge of the badge.


(3) Titles shall be as follows:
(i) A Medical Doctor shall have the title "Physician."
(ii) A Doctor of Osteopathy shall have the title
"Physician."
(iii) A Registered Nurse shall have the title
"Registered Nurse."
(iv) A Licensed Practical Nurse shall have the title
"Licensed Practical Nurse."
(v) All other titles shall be determined by the
department. Abbreviated titles may be used when the title
indicates licensure or certification by a Commonwealth
agency.

(4)A notation, marker or indicator included on an identification badge that differentiates employees with the same first name is considered acceptable in lieu of displaying an employee's last name.



Observations:

Based on observation and interview with the Agency Administrator, it was determined that the Agency failed to ensure a photo identification tag that would include the employee's full name, title, picture and the name of the agency for office employees.

Findings include:

During observation on July 21, 2021, at approximately 11:30 a.m., the agency Administrator, and other personnel was not wearing a photo identification tag.

In an interview on July 21, 2021, at approximately 2:15 p.m., the agency Administrator confirmed that photo identification tags had not been initiated for the agency.








Plan of Correction:

All Good Brothers Home Health Care employees will be given identification badges with their names, job title and agency name in clear sight and instructed to wear during business hours at all times. Going forward all future employees will have identification badges at the start of their first employment date to wear when working with our participants. Current employees will be provided with identification badges as well.
